If you’re interested in one of our upcoming puppy litters please fill out this waiting list application form. I am very careful about placing my puppies in responsible homes so here are some questions that will help me get to know more about you.

I hope you will agree that the animal’s welfare must be my foremost consideration in considering a placement.

Willowbranch Canine Companions requires new owners to commit to contact us if you are not able to continue to care for your dog. Are you committed to returning your puppy/dog to Willowbranch Canine Compainons? I agree that if at any point I cannot keep the puppy, I will return him / her to Willowbranch Canine Companions without requesting compensation, and I will pay all shipping costs prior to shipment.(You may be asked to pay flight charges for shipping) *YesNo

Price: $1,200 - $2,200. Flights are an additional $500-$875 if the puppy is shipped at 8-10 weeks, higher if puppy is older. The shipping includes airfare and being hand carried by our flight nanny to the airport of your choosing or crate and health certificate for cargo shipping. Shipping costs outside the United States and ground transportation delivery will be discussed upon the first deposit. A non-refundable deposit of $250 ($700 for Sheepadoodle and Bernedoodles) is required to reserve the puppy you choose, which will be applied to the purchase of said puppy.The balance is due 1 week (7 days) before delivery or pick up. We accept Venmo or Cash for deposit. When paying the final amount Cash is the preferred payment unless otherwise arranged . We Reserve the right to take pick of the litter on any litter for our Breeding Program. We do not take a breeders pick with every litter. Breeders Pick is before any customer first pick. We are an Iowa State licensed business and therefore must charge 7% Iowa State tax for any puppies picked up in Iowa. Do you agree to these terms? *YesNo

I understand that if the puppy is killed in an accident (plane crash or car accident) before delivery of the puppy to you, Willow Branch Canine Companions will reimburse you for the money you paid for the dog, but will not reimburse you for the plane ticket, health certificates, crates bought and money for driving to or from airport. I agree that the puppy is being adopted for myself and will not be sold, adopted, or given to another party. The buyer will pay all costs of transportation to return the puppy/dog back to Willow Branch Canine Companions. There will be no compensations of purchase price of puppy/dog. *YesNo

I agree that only under proper supervision will the puppy be allowed outdoors. When taking my puppy outdoors, they will be on a secure harness and wear proper ID. I also agree that this animal will be a companion animal only (no guard dogs or breeding dog). I agree that at no time will this dog be kept on a chain or rope or live outside *YesNo

I understand and agree that Willow Branch Canine Companions makes no guarantees about the dog’s temperament, size, color or amount of shedding. Willow Branch Canine Companions is not responsible for future damages or injuries caused by the dog. I agree to contact Willow Branch Canine Companions if my companion dog needs to be euthanized for any reason, prior to being euthanized (unless the dog’s veterinarian determines this is a medical emergency at which time I will still contact Willow Branch Canine Companions). *YesNo

I agree to care for this dog in a humane manner and to be a responsible animal guardian. This agreement includes adequate food, water, attention and medical care. I also agree to give proper bedding and covering from the elements. All this will be done at my expense without charging Willow Branch Canine Companions for these expenses. *YesNo

I agree to take my puppy to the vet for a complete and thorough check no more than 3 days after adopting this puppy. A receipt is required to be sent to Willow Branch Canine Companions, of the puppies wellness check. I also agree that I will provide Willow Branch Canine Companions proof spay or neuter at the age appropriate time 6-12 months. A $500 assessment will be charged to me if this is not completed. I will email or message these proofs to Willow Branch Canine Companions at mikkizook@gmail.com with the puppy's name and date of birth in the subject line. Willow Branch Canine Companions has made every effort to provide you with a healthy, happy companion puppy/dog. Willow Branch Canine Companions is not responsible for any financial obligations that may occur due to the health after adoption including common puppy illnesses such as parasites, coccidia, giardia, UTI, puppy influenza, parvo or any other illnesses treatable with medication. *YesNo

I agree to spay (female) or neuter (male) the puppy by 1 year old and send proof of spay or neuter from my vet by way of receipt. If I choose not to spay or neuter the puppy I adopted from Willow Branch Canine Companions, and send a receipt from my vet in the time frame requested, that I have spay or neutered my puppy, then Willow Branch Canine Companions has the right to actively pursue a fee of $5,000 for fees charged for a "breeding dog". In the event of a life threatening congenital defect being diagnosed within the 1st year, we will replace the puppy with a new puppy of the same value. No monies shall be exchanged unless you choose a new puppy of more value. Willow Branch Canine Companions is not responsible for any veterinarian cost. This 1 year guarantee is for the health of your puppy that has been adopted from Willow Branch Canine Companions, and does not cover accidental death, neglect, abuse, or injuries that your dog may sustain while being in your ownership. If for any reason I choose to take Willow Branch Canine Companions to court, I agree to file the motion in Henry County, which is the county from which my puppy was purchased.
